Consistent Senior Care Manager involvement built around your loved one’s everyday life.
Private In-Home Senior Support is an ongoing monthly service with regular scheduled time between your loved one and their Senior Care Manager. The time is centered around everyday life while helping your loved one remain independent, active, organized, connected, and involved in the routines and activities that matter to them.
Scheduled time includes everyday organization, meals and hydration, medication routines, technology, appointments, errands, hobbies, activities, and time in the community. Your Senior Care Manager also pays attention to mobility, memory and communication, mood, nutrition, daily functioning, and changes within the home.
After each session, your Senior Care Manager documents important observations, keeps authorized family informed, updates relevant records, and prepares for the next scheduled session.
A Personal Care Portfolio and secure Client Portal keep important information, updates, schedules, and records organized throughout the relationship.

Private Senior Care Coordination includes that direct involvement and adds active coordination across healthcare providers, appointments, recommendations, services, hospital or rehabilitation stays, family communication, and larger care decisions.
We recommend the level of involvement that fits your loved one’s current needs. As those needs develop, the same Senior Care Manager relationship continues with the level of support adjusted around them.
Private Senior Care Coordination includes everything provided through Private In-Home Senior Support and adds dedicated professional time for coordinating the healthcare, providers, appointments, services, recommendations, family communication, and responsibilities surrounding your loved one’s care.
Your Senior Care Manager prepares for and attends important healthcare appointments, communicates with authorized providers, tracks referrals and provider recommendations, follows provider-directed medication changes, and coordinates services. After appointments, we carry the information forward by organizing follow-up, testing, referrals, home health, therapy, equipment, and other care that becomes part of your loved one’s plan.
As needs develop, the same Senior Care Manager relationship continues through emergency room visits, hospital or rehabilitation stays, the return home, home health, therapy, hospice, increasing care at home, and larger family decisions. When assisted living or memory care becomes part of the plan, we research appropriate communities, coordinate tours and transition details, and the Care Coordination relationship remains available after the move.
Throughout the relationship, your Senior Care Manager keeps authorized family informed, brings important questions and observations into the right conversations, and keeps your loved one’s preferences, goals, and priorities visible. A Care Coordination Plan, Personal Care Portfolio, and secure Client Portal keep current providers, appointments, recommendations, priorities, responsibilities, and important updates organized as the situation develops.
Janis
Senior Care Manager
Janis brings more than 12 years of hands-on healthcare experience working directly with older adults and individuals with complex health and neurological conditions. Her background spans medical care, long-term care, home-based care, memory care, hospice, medication support, and direct patient care.
She has spent years working closely with people whose needs range from relatively independent to medically and physically complex, giving her a strong understanding of how health conditions, medications, mobility, cognition, daily routines, and changes in function can affect someone’s life and the care surrounding them.
She brings that healthcare knowledge into every Senior Care Manager relationship, helping families recognize important changes, prepare for healthcare conversations, and understand how different parts of a loved one’s health and daily life connect.

Senior Care Manager
Jennifer brings more than 20 years of experience helping people through situations that require organization, communication, sound decision-making, discretion, and steady follow-through. Her experience includes coordinating complex responsibilities, working across multiple professionals and organizations, supporting people during highly emotional circumstances, and helping families make sense of what needs to happen.
Caregiving has also been a significant part of Jennifer’s personal life. She has cared for both of her parents and has long been someone family and friends turn to during hospitalizations, recovery, health changes, and times when another person is needed to help keep everything moving.
Together, Jennifer’s professional and personal experience gives her a strong understanding of what families are managing and how to keep people, information, responsibilities, and follow-through organized as care becomes more involved.
