Private In-Home Senior Support, Private Senior Care Coordination, client portals, assessments or planning completed as part of a client engagement, and other client relationships may be governed by separate service agreements, authorizations, policies, and notices.

  • A contractual obligation to provide services

A formal client relationship begins only after West Michigan Senior Care has:

  • Reviewed the circumstances

  • Determined that the requested service is within our scope and current availability

  • Completed any required intake, assessment, or onboarding process

  • Accepted the engagement

  • Received required agreements, signatures, authorizations, and payment arrangements

  • Completed any other requirements necessary to begin services

Until those steps are completed, do not rely on West Michigan Senior Care as responsible for monitoring, managing, or responding to an individual's care needs.

6. Services and Initial Conversations

Descriptions of Private In-Home Senior Support, Private Senior Care Coordination, and other services or features on this website are general summaries. The specific work provided to an individual client is determined by the applicable service agreement, assessment and planning process, authorizations, scheduling, and the circumstances of the engagement.

Actual services may depend on:

  • Individual circumstances and needs

  • Geographic location

  • Availability

  • Scheduling

  • Selected service

  • Scope of engagement

  • Required authorizations

  • Safety considerations

  • The terms of the applicable service agreement

A complimentary initial conversation allows us to learn about the circumstances surrounding an older adult and family, answer preliminary questions, explain our services, and determine whether West Michigan Senior Care may be an appropriate fit.

A complimentary initial conversation does not create a client relationship and does not include a formal assessment, individualized care or coordination plan, medical diagnosis or treatment, legal or financial advice, guaranteed placement, or an obligation by either party to begin services.

We may decline or discontinue an inquiry when the requested assistance:

  • Is outside our service scope
  • Requires services we do not provide
  • Presents safety concerns
  • Requires emergency intervention
  • Cannot be reasonably accommodated
  • Would create a conflict of interest
  • Is otherwise inappropriate for the business to accept

7. Website Forms and Communications

You are responsible for ensuring that information submitted through the website is accurate, lawful, and appropriate.

General website forms and ordinary email may not be secure channels for highly sensitive information.

The involvement of West Michigan Senior Care does not guarantee a particular outcome, and we are not responsible for the independent acts, omissions, decisions, availability, or performance of third-party providers, facilities, agencies, or professionals.

14. Limitation of Liability

To the fullest extent permitted by law, West Michigan Senior Care and its members, employees, and agents shall not be liable for any direct, indirect, incidental, or consequential damages arising out of your use of the website or the informational content provided herein.

15. Indemnification

You agree to indemnify and hold harmless West Michigan Senior Care from any claims, losses, or damages resulting from your violation of these Terms or your misuse of the website.

The parties retain the right to seek emergency injunctive or other equitable relief in a court of competent jurisdiction to prevent the actual or threatened infringement, misappropriation, or violation of a party's copyrights, trademarks, trade secrets, patents, or other intellectual-property rights.

20. Entire Agreement

These Terms and our Privacy Policy constitute the entire agreement between you and West Michigan Senior Care regarding your use of the website.

21. Severability

If any provision of these Terms is found by a court or arbitrator to be invalid or unenforceable, the remaining provisions will remain in full force and effect.

22. No Waiver
