Service Scope

West Michigan Senior Care Management provides in-home support and assessment visits, ongoing support visits, care management, care coordination, placement and transition support, family guidance, and related support services for older adults and their families. Our services are designed to help families better understand care needs, identify concerns, coordinate support, and make informed decisions regarding aging-related care and life transitions. We do not provide medical treatment, nursing services, personal care, medication administration, legal services, financial planning, transportation services, or emergency healthcare services.

Scheduling and Payments

All support sessions must be booked in advance. Detailed pricing, payment procedures, and service specifics are disclosed during the booking.

Ongoing care management, specialized project-based tasks, and initial consultations may involve different rates based on the depth of coordination needed.

Client Responsibility

Clients must provide truthful and current details regarding their loved one's health history, immediate care needs, and the specifics of their current living setup.

Every final decision regarding health, legal matters, or finances remains the duty of the client and family.

Third-Party Services and Recommendations

West Michigan Senior Care Management may recommend community resources, healthcare providers, home care agencies, senior living communities, contractors, moving services, or other third-party organizations.
While we strive to recommend reputable providers and resources, we do not own, operate, supervise, or control third-party organizations and cannot guarantee the quality, availability, performance, or outcomes of services provided by others.
Families are encouraged to independently evaluate any provider or service before making decisions.

Cancellations and Rescheduling

We recognize that care needs evolve rapidly. If a scheduled consultation needs to be moved or canceled, we request that you provide us with reasonable notice.

Specific policies regarding rescheduling or cancellations may be detailed at the time of service selection and booking.

Limitation of Liability

Through professional guidance, communication, and coordination, West Michigan Senior Care Management assists with navigating needs, but cannot guarantee results for third-party facility placements or specific outcomes of individual health plans.

Our function is to provide expert recommendations and support to empower families in their decisions.

Emergency Situations

Our services are not intended to replace emergency services, healthcare providers, or urgent medical care. If there is an immediate safety concern, medical emergency, mental health crisis, suspected abuse, or urgent healthcare need, call 911 or contact the appropriate emergency or healthcare professional immediately.

No Guarantees

West Michigan Senior Care Management provides guidance, support, recommendations, and coordination based on information available at the time services are provided.
Because every individual, family, healthcare situation, and care environment is different, we cannot guarantee specific outcomes, results, healthcare decisions, placement availability, or future care needs.
