We take the time to understand how they live, what matters to them, what may be changing, and what your family is managing. Our involvement can extend wherever it is most useful—at home, in the community, at healthcare appointments, through hospital or rehabilitation stays, and during larger care decisions.
As more becomes involved, you have an experienced Senior Care Manager who already understands the history and can help keep the different parts of care connected.

Aging rarely affects just one part of life. A change in mobility can affect getting out of the house. A medication change can affect how someone feels during the day. A hospital stay can lead to therapy, new appointments, new instructions, or more help at home. And sometimes the biggest need is simply having someone who knows your loved one well enough to recognize when everyday life is beginning to look different.
Our role is designed to connect those pieces.

We spend real time with your loved one and learn how they actually live—the routines they value, the activities they enjoy, how they manage meals and medications, what they like doing for themselves, where something has become harder, and what helps them continue living life in a way that feels familiar.
That might mean sharing lunch, working in the garden, getting out into the community, helping with a routine, preparing for an appointment, or working through something at home that deserves attention.
What happens at home can matter during a medical appointment, and what happens during an appointment can affect everyday life afterward.
We can help bring useful history, questions, and observations into healthcare conversations, attend appointments, help your family understand recommendations, and connect those recommendations back to what is actually happening in your loved one’s daily routine.
When more coordination is needed, that involvement can extend across physicians, specialists, therapy, home health, medication changes, referrals, and other professionals involved in their care.
Family, Resources & Additional Support
Sometimes the answer is not simply “more care.” It may be a practical change at home, a community program, therapy, transportation, a meal resource, additional hands-on help, legal or planning guidance, or another professional who should become involved.
We help your family think through what would actually be useful, identify appropriate resources, and keep those added pieces connected to the larger situation.
Hospitalizations, Rehabilitation & Larger Changes
If your loved one is hospitalized, enters rehabilitation, begins receiving more care at home, experiences progressing memory changes, starts hospice, or eventually moves to assisted living or memory care, West Michigan Senior Care can remain involved.
Your Senior Care Manager can carry forward the knowledge already built about your loved one—their history, routines, preferences, family, providers, and priorities—and use that understanding as new decisions and new people become part of their care. The Care Coordination model is specifically designed to expand around hospitalizations, rehabilitation, therapy, hospice, dementia progression, and senior-living transitions.
Having an experienced Senior Care Manager involved gives your family someone who knows your loved one, understands what matters to them, pays attention to the details of everyday life, and stays connected as different parts of life and care require attention over time.

Private In-Home Senior Support centers on consistent involvement in everyday life. Private Senior Care Coordination adds dedicated responsibility across healthcare, services, transitions, family communication, and the broader care situation.
Private In-Home Senior Support
Consistent Senior Care Manager involvement in the life your loved one is already living.
Private In-Home Senior Support is ongoing monthly support centered around regular, dedicated time with Janis or Jennifer.
We get to know your loved one as a person—their routines, interests, preferences, relationships, abilities, and the things they want to continue doing. Our time together is shaped around real life and may include meals, activities, errands, appointments, routines at home, time in the community, or something that has become more difficult and would benefit from an experienced person alongside them.
Consistent time together gives us valuable context.
Because we are involved consistently, we develop a real understanding of how your loved one normally moves through their day. We notice patterns, recognize when something begins to look different, think about what could make everyday life work better, and give your family another experienced perspective on how they are doing.
That understanding can also become useful beyond the home. If an appointment is coming up, something has changed, or another professional becomes involved, we already have context about your loved one and their everyday life that can help your family ask better questions and make more informed decisions.
Private Senior Care Coordination
When your family needs someone experienced taking responsibility across more of the care picture.
As more healthcare professionals, appointments, services, recommendations, and decisions become involved, keeping everything connected can become a significant responsibility of its own.
Private Senior Care Coordination includes the direct relationship and everyday involvement provided through Private In-Home Senior Support, while expanding your Senior Care Manager's role across the larger situation.
That may mean preparing for and attending important healthcare appointments, communicating with authorized providers, tracking referrals and recommendations, helping coordinate therapy or home health, staying involved through a hospitalization or rehabilitation stay, helping carry out discharge plans, bringing family members into important conversations, or helping your family prepare for a future move.
It also means connecting what is happening in different places.
A change we notice at home may be important information for a physician. A therapist's recommendation may need to be worked into your loved one's actual routine. A hospitalization may create several things that need attention once they return home. When multiple people are involved, your Senior Care Manager is looking at how those pieces affect one another and what still needs to happen.
Private Senior Care Coordination gives your family an experienced Senior Care Manager who can take an active role across healthcare, services, transitions, and other responsibilities while staying grounded in what is actually happening in your loved one’s everyday life.

When it appears that we may be a good fit, the next step can be a complimentary in-home introduction. This gives us the opportunity to meet your loved one, meet involved family members, understand the general home setting, talk more specifically about what your family is looking for, and make sure everyone feels comfortable with the relationship.
Begin the Ongoing Relationship
Once your family decides to work with West Michigan Senior Care, we agree on the starting service and schedule your first session.
Your first session includes the Initial Assessment & Client Setup, where we learn your loved one in greater depth, establish the starting baseline, begin the Care Portfolio and Client Portal, and start the ongoing work your family has hired us to provide.
The Initial Assessment & Client Setup is included within your chosen ongoing service.
